Patients report their symptoms through our built-in evidence-based guidelines, then request a video conference with the provider for a more in-depth consultation. Upon receiving notification of a new patient case, the assigned clinician can log on using the Connect N’ Care® app or web-based portal and review the SOAP notes, then proceed to do a quick video consult so they can “lay eyes” on the patient. At the end of the consult, the telehealth encounter can then be documented in the EMR using API’s, HL7 or FHIR protocols.
