An elevated troponin level (above the threshold) within 4-12 hours of acute myocardial ischemia is highly specific and sensitive for the diagnosis of acute myocardial infarction. According to the ESC and ACC Joint Protocol - Redefinition of Myocardial Infarction, which gives a definition of myocardial infarction and a description of the reliability of the hs-cTnl assay, AMI is defined as any condition in which the troponin I concentration exceeds the standard 99% confidence interval threshold of the reference control group. Therefore, the acceptable (total) imprecision of each measurement at the 99% confidence interval should be defined as no more than 10%. Troponin concentrations peak at 14-36 hours of acute infarction and can remain high for 7 days. A series of tests for ultrasensitive troponin I concentrations is recommended for patients with suspected myocardial injury. Also, hs-cTnI is the marker of choice for risk stratification and should be performed in all patients with suspected ACS, if feasible. Patients with clinical symptoms consistent with ACS and peak hs-cTn above the 99th percentile of the normal reference population are at increased risk for death and ischemic event recurrence.
