The main cause of Apical and Periapical diseases is the presence and/or passage of toxic substances from the endodontium to the periodontium. Therefore, the main aim of endodontics is the perfect and TOTAL disinfection of the endodontium.

The increasingly faster update of surgical techniques and the worldwide spread of informational literature on more or less recent experiments and studies confirm that Sodium Hypochlorite (NaOCl) is considered as the best root canal irrigant for its antibacterial, collagen-dissolving and lubricating properties.
When Sodium Hypochlorite comes into contact with watery solutions, it dissociates in Sodium Hydroxide and Hypochlorous Acid (HOCl) according to the following chemical reaction: NaOCl + H2O → NaOH + HOCl.
Hypochlorous Acid is a powerful germicide with a wide antibacterial spectrum and is able to kill Gram+ and Gram- bacteria, sporulating micro-organisms, (clostridia and fungi) and protozoa, that is, the most common micro-organisms of the endodontic flora.
At the end of the Seventies, several researchers experimented in vitro that, regardless of the various concentrations considered and generally used (0.5%, 1%, 2.5%, 5%, etc.) according to the known chemical and physical law, HEATING Sodium Hypochlorite increased the speed of production of Hypochlorous Acid, thus encouraging the use of Sodium Hypochlorite.
