Description

The supra-acetabular part of the iliac bone transmits weighting forces from the hip to the sacro-iliac joint and spine. Osteolytic metastases limited to the supra-acetabular area can be treated by cementoplasty which increases compression bone strength. However, when bone destruction extends to the arcuate line of the iliac bone and toward the sacroiliac joint, where shearing forces are exerted, cementoplasty is not sufficient to restore bone strength. In such situations, a nail introduced obliquely through the posterior inferior iliac spine toward the supra-acetabular region, along the thickest part of the iliac bone, is able to substitute the sustain forces from the hip to the sacro-iliac joint. The nail is stabilized by fixing it by two screws placed posteriorly into the sacral ala through the sacroiliac joint and one screw placed into the anterior part of the nail into the supra-acetabular region. Finally, polymethylmetacrylate cement is injected through the nail for further bone fixation. Indication : Extensive tumoral destruction of the supra-acetabular iliac bone including the arcuate line with preservation of the hip joint space and sacral ala. Contraindication: The use of iliac nail instrumentation is contraindicated in patients with open wounds, infection, pregnancy, and osteolytic tumors destroying the hip joint space and sacral ala.
