The ungual prosthesis is slightly larger than normal in order to enable resizing and adaptation to all nail forms. Its concavity reproduces the form of a normal nail. The fundamental quality is that its underlying side is striated longitudinally like a natural nail. The nail must be attached laterally by two separate points, or with a U-point
On the other hand, it is not advised to place a point in the dorsal skin at the risk of arriving in the matrix and leaving a permanent longitudinal ridge . This setting technique advocated by certain authors must be done away with.
Moreover, the prosthesis will only be inserted under the skin during “cold” surgery, in reconstructing the groove of the matrix.
In urgent case, we suggest inserting a gap between the dorsal skin and the sub-cutaneous part of the nail (soft nail), which decrease the risks of periungual panaritium. Even if a prosthesis delivered sterile is better tolerated than an avulsed nail, it is imperative to insure proper drainage of the hæmatoma.
