1) Aiming • Drill through the external cortical at the chosen point. • Drive the pin (of 20 or 25/10) with a protractor, generally at 130°. • After about 60 minutes, replace the protractor with the pin guide and finish driving the pin. The difference between the pin (200 mm) and the guide (100 mm) allows to easily assess the wire’s penetration. • An X-ray control may be useful to check the wire’s status. 2) Drilling • Stop about 10 or 15 mm before the pin’s extremity in order to: 1) maintain it steadily. 2) avoid getting it stuck. 3) get a better hold of the nail. • Drill with the small auger drill with motor (7 mm). • Remove the motor, while keeping the small drill and the pin in position. • Bore out the external cortical with the bigdrill (14 mm), which threads on the little one. 3) Calibration In many cases (i.e. hard or irregular bones), it is worth correcting the housing with the nail-starter, whose inferior side must be maintained in the diaphysis plane. 4) Nailing • Set the nail-plate on the whole nailholder. Screw the male part all the way and screw it again after the first hammer- strokes. • While driving in, make sure to maintain the plate in the diaphysis axis. •Do not use the impactor at this time. 6) Diaphyseal setting • Diaphyseal screws: 5 mm or 4.5 mm Ø. • Only use the impactor after screwing (to avoid the nail sliding upwards). • Eventually finish with a screw in the oblique hole, especially if the nail is a little bit too high, or in order to stabilize the lesser trochanter.
